When he's not singing, playing instruments, or just being totally adorable, Nick Jonas is SERIOUS about raising diabetes awareness. For Nick, the cause hits close to home—in 2005, he was diagnosed with type 1 diabetes.

Last summer, Nick teamed up with Bayer Diabetes Care to create cool dog tags for people with and without diabetes for a program called Nick's Simple Wins. "Simple wins" are personal accomplishments that are meaningful to you.

We're giving away FIVE of these limited-edition dog tags to lucky Club BSGers. Nick wears his constantly!
